Your Rights Regarding Your PHI

You have the right to:

  • Request Corrections: Ask us to fix incorrect or incomplete information.
  • Request Restrictions: Ask us not to use or share certain information for treatment, payment, or operations.
  • Request Confidential Communications: Ask us to contact you in a specific way or at a certain location.
  • Receive a List of Disclosures: Request a list of when and with whom we’ve shared your PHI, except for routine uses.
  • File a Complaint: If you believe your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health & Human Services. We will not retaliate against you for filing a complaint.

  • We must provide you with this notice about our privacy practices.
  • We must follow the terms of this notice.
  • We will notify you if there is a breach of your unsecured PHI.

Changes to This Notice

We may update this HIPAA policy from time to time. When we do, we will revise the “Last Updated” date at the top of this page. Updates will be posted on our website and available upon request.
